Malaria : Drug as effective in low doses

A new study finds that lower doses of the drug primaquine are as effective at preventing transmission of malaria as standard doses.

Primaquine targets the transmission stages of the malaria parasite, the gametocytes, and is considered an important tool for malaria elimination .

The new study, led by researchers from the London School of Hygiene & Tropical Medicine (LSHTM) in the UK, is published in the latest online issue of The Lancet Infectious Diseases.

The findings are good news because there are not many drugs that target this stage of the disease, and unfortunately, at standard doses primaquine can reduce blood count in people with a red blood cell disorder caused by a deficiency in an enzyme called G6PD.

The inherited blood disorder is common in places where malaria is endemic and so has limited the use of primaquine in those areas.

Because of this side effect, the World Health Organization (WHO) advised that dosage of primaquine be reduced from 0.75 mg/kg to 0.25 mg/kg. However, until now, nobody had formally evaluated the effectiveness of the drug at lower doses.

Lead author Dr. Alice Chi Eziefula, Wellcome Trust clinical research fellow at LSHTM, says:

"Until now, the use of primaquine was limited because of safety concerns, but lower doses had never been tested formally."

Half of standard dose just as effective
The study took place in Jinja, Uganda, where children without the red blood cell disorder were treated with a malaria drug either on its own or with one of three different doses of primaquine.

The children were then monitored for 2 weeks to test their malaria gametocytes, and they were kept under observation for another 2 weeks.

The resuls showed that a dose of 0.4 mg/kg of primaquine, or about half of the previously recommended (standard) dose, was as effective at reducing the transmission potential of individuals with malaria.

Dr. Chi Eziefula says:

"More questions remain to be answered regarding the best operational strategy for the deployment of primaquine to block malaria transmission."

The next step is to evaluate the effectiveness of the current WHO-recommended 0.25 mg/kg dose of primaquine in people without the red blood cell disorder, and then to test the safety of such a dose in people with the blood disorder.

Co-author Moses Kamya, a professor with the Infectious Disease Research Collaboration in Kampala, Uganda, says:

"This study provides important contemporary information that allows malaria control programmes in endemic countries in Africa to consider the use of primaquine as part of their efforts to deal with this killer disease."

Sotting the signs of serious illness

Headaches are extremely common and most of them aren't a cause for anxiety. Occasionally, though, a headache is a symptom of a serious illness, such as meningitis or a brain haemorrhage, and requires urgent medical attention. Tension headaches are the most common type. What are the causes?
About three out of four headaches are caused by tension in the scalp or neck muscles due to stress. Tension headaches tend to occur frequently and cause moderate pain, particularly at the back and front of the head. It's often described as a tight band encircling the head. Other common causes of headaches include hangovers, having irregular meals, long journeys, noise, a stuffy atmosphere, thundery weather, too much sleep, too much excitement, a fever, sinusitis and toothache.
A migraine is a one-sided severe headache with eye symptoms and possibly vomiting and can run in families. A severe headache with fever, a stiff neck and rash may be a sign of meningitis, a condition in which the membranes covering the brain and spinal cord become inflamed.
A sudden headache that feels like a blow to the back of the head could be a subarachnoid haemorrhage, in which bleeding occurs between the membranes covering the brain.
In elderly people, a headache with tenderness of the scalp or temple may be due to temporal arteritis, in which blood vessels in the head become inflamed.
What might be done?
If your doctor suspects an underlying condition, you may require tests, such as CT scanning or MRI of your brain, and an opinion from a neurologist.
What's the treatment?
The treatment depends on the cause of the headache. For example, a tension headache will usually clear up with rest and painkillers. Cluster headaches and migraines can be treated with drugs. Excess painkillers, especially those containing codeine, can actually cause a headache.

Robot talks in 19 languages!

Designed for companionship, a new toddler-sized robot can hold a conversation in 19 different languages. Built by Paris-based robotics company Aldebaran and infused with language-learning software developed by voice-technology company Nuance, the robot has a voice that doesn't sound much older than it looks. The android can walk, brace itself if it falls and will develop its own personality as it gets better at speaking through repetition.
"He lives his life on your side at home," Marine Fabrea, Aldebaran's head of communications, told Mashable in an email. " You can discuss with him, you can ask him to tell a story to your kids."
The difference between these robots and mindless machines is built-in emotional intelligence, according to Arnd Weil. Nuance's vice-president and general manager of automotives.
"What they are trying to do with the robot is mainly capturing what is the mood of that other person," he said. "It's more like somebody's coming home and he had a bad day, he's angry, so the robot should capture that.... and react accordingly."
The robot will be able to access a cloud of data that builds its vocabulary, as well as help it understand the flow of conversation, Weil added; this way, it can better react to users the more they interact with it, much like how Nuance's text-reading app develops language skills.
Weil foresees the robots bringing out drinks, chatting with people who are waiting for appointments, acting as highly functional pets, and providing companionship to the elderly or children with disabilities.
"People do need somebody to talk to," he said. " A lot of people have animals or a dog or something, and this is just a new way of engaging with people."
The robots will be available next year.

Sex is not the secret to lasting love

Scientists have claimed that sex and having good communication can have a significant influence on relationships but factors like knowing partner's favourite pizza topping, being employed and ability to support yourself also matter. Scientists tested 2,201 participants, head to head, in seven "relationship competencies" that previous researchers had found to vital for promoting happiness in romantic relationships.

The researchers idea was to rank them in order of vitality to start building data on the aspects of relationships most important to keep them healthy.
The researchers, in addition to communication and conflict resolution, also looked out for love-making or romance, stress management, life skills, knowledge of partners and self-management to find the best predictors of relationship satisfaction.
Couples were queried about their competency in these areas and then asked how satisfied they were in their relationships; the researchers then correlated each partner's strengths and weaknesses in each area with the relationship satisfaction of the person.
Couple, reporting communicating effectively, showed highest satisfaction with their relationships.
Study's lead author Robert Epstein, a professor of psychology at the University of the South Pacific, in Fiji, said that learning more about partner is also important for a successful relationship. Time Magazine reported.
He added that even more important was to know critical things like whether partner wants kids.

Facebook expands ad service for app developers

Facebook Inc (FB.O) is expanding an advertising service aimed at developers of smartphone and tablet apps, the social network's latest move to bolster it's fast-growing mobile business.
Facebook's so-called mobile app ads - which allow companies to promote their mobile apps to Facebook's 1.15 billion users - will include new capabilities designed to boost the amount of time that consumers use the third-party apps, the company said on Tuesday.
Since the start of the year, more than 145 million third-party apps have been installed by Facebook users, said Deb Liu, Facebook's director of platform monetization, providing the company's first public data about that portion of it's business. Roughly 8,400 advertisers used Facebook's mobile app ads in the second quarter, Liu said.
That is a small group relative to the more than 1 million total advertisers that Facebook counts, but analysts say that app developers represent an attractive and growing group of customers for Facebook to tap into. Smartphone apps, designed for Apple Inc's (AAPL.O) iPhone or Google Inc's (GOOG.O) Android-based phones, have struggled to stand out in a crowded field.
Facebook's new mobile app ads will allow the maker of a travel app to offer to promote an airfare or hotel room, for example, while a music app like Spotify could promote a band's new song. Previously, Facebook's mobile app ads were designed only to entice its users to install a third-party app on their smartphone or tablet, but not to encourage repeat visits to the app.

With apple power, battle cancer and cut calories

"An apple a day keeps the doctor away" is certainly no exaggeration, for the mighty apple is indeed a storehouse of vitamins, minerals and many other valuable substances. In fact, apple consumption has been associated with reduced risk for a number of diseases including cancer, particularly lung cancer, as well as cardiovascular diseases, asthma and type 2 diabetes. Apples are rich in vitamin C and vitamin E, the content varying with the 7,000 known varieties. Vitamins C and E are powerful antioxidants, which help to build immunity. fight chronic diseases and slow down ageing.
Apples are also an excellent source of disease-fighting soluble fibre in the form of pectin. Its high pectin content helps lower cholesterol and acts as a laxative by bulking the stool and maintaining intestinal balance. Pectin also helps to bind toxic metals such as mercury and lead, and helps excrete them.
Cooked apples are known to relieve diarrhoea, dysentery and colitis. One large apple a day has been shown to reduce serum cholesterol levels by 8-11 percent.
The pectin in apples supplies galacturonic acid to the body, which lowers the body's need for insulin and may help in the management of diabetes. Apple pectin helps in gelling and it is this property which helps in making apple jams and jellies.
Apples and apple juice are rich in phytochemicals (plant chemicals) including flavonoids and polyphenols. Polyphenols such as tannins (tannic acid) are potent enemies of viruses particularly herpes simplex virus. Two polyphenols - phloridzin and phloretin xyloglucoside found in apples have not been found in any other fruit.
Not only are apples exceptionally rich in phenols, they also have the highest concentration of "free phenols", which means that they are more available for absorption into the blood stream. IPL7 may move to Bangladesh, Sri Lanka -BCCI

The next season of the Indian Premier League (IPL) - or at least some matches of the T20 tournament may be played outside India. Sri Lanka or Bangladesh, or both, could be the likely host, a top cricket board official said.
The reason is the same as in 2009, when the second season was played in South Africa: Lok Sabha elections. The election dates are not known yet, but the BCCI has begun to prepare for a possible overlap with the IPL, which is played in April and May.
"A clear picture will emerge only after the dates for the Lok Sabha elections are announced. Until then, we are considering whether to have a few games in India and then move abroad, or vice versa. Both Bangladesh and Sri Lanka are short journeys, so logistically, it won't be much of a problem," the BCCI offcial said.
The current Lok Sabha's term ends in May 2014, and if the election is not brought forward, the IPL will coincide with  campaigning and voting, and severely stretch security forces.
"The decision to move to South Africa in 2009 was taken at the last minute, and nobody had expected than that there would be an issue regarding security. But we now know that such a situation may arise again, and we are getting ready for it," the official said.
IPL CEO Sundar Raman will come up with detailed plans in due course. The BCCI is considering splitting season 7 into two halves, one of which might travel outside India.
Unlike in 2009, the board will not this time knock on the doors of Cricket South Africa (CSA) - the reasons being the recent tensions with their CEO Haroon Lorgat, and the expensive dollar, which would make playing the tournament in South Africa Significantly more expensive.

When we did that, we could release it on Eid," he said.
While making a film, SRK is not bogged down by any thought and added: "When I am doing a film, I don't think or anything else."
He hopes that everyone earns profit from "Chennai Express".

Government to procure 650 taxicabs with help from army

Communications Minister Obaidul Quader said the government will provide 650 new taxicabs to Dhaka and Chittagong. Dhaka will get 250, Chittagong 250 and army will supervise 150 cabs.

The Minister said the decision came at the annual development programme (ADP) review meeting on Wednesday at Communications Ministry.

The Minister said considering Dhaka's transportation system, a decision has been taken to give route permits in Dhaka to AC minibuses only.

He said the Prime Minister will lay the foundation stone of the Metro Rail, Dhaka Sustainable Urban Transport Project and second Kanchpur, Meghna, Gomoti bridge construction next month.

Obaidul Quader said the Tk. 22,000 crore project will be implemented under Dhaka Mass Rapid Transit Development Project. Of the total cost, Japan International Cooperation Agency (JICA) will provide Tk. 16,600 crore.

Government prepares for execution

Government is preparing to execute Jamaat leader Abdul Quader Mollah's death sentence. Government believes there is no scope for review appeal against the apex court's verdict. In fact, there is even no need to wait for the full verdict. Jail authorities can start the procedure once it receives the certified copy of the abridged verdict. But is this case Quader Mollah will get a week to seek presidential clemency. Meanwhile, Quader Mollah's counsel has applied for a certified copy of the abridged verdict. They have appealed for a stay on the execution procedure stating a review petition will be filed as soon as the copy of the verdict is received.
Executions are processed under penal codes and jail codes. The jail codes mandate that after receiving the verdict the authorities allow 1 week for the convict to seek presidential pardon. If the convict is not pardoned then the sentence is executed between 21-28 days. But these penal codes and jail codes will not be applicable to war crimes convicts because International Crimes (Tribunals) Act-1973 is a special law. The sentences in this case will be executed as per the government's orders.
The Appellate Division allows 30 days to file any review petition. Quader Mollah's chief defense counsel said yesterday filing a review petition is Mollah's constitutional right. However, law experts believe Section 105 of the Constitution does not apply to war crime convicts and as such there is no scope for review in Mollah's case. Both Law Minister and Attorney General believes there is no scope for a review of the verdict.
Defense counsel on the other hand believes execution of the verdict without first allowing to file a petition would make it a judicial killing. The jurisprudence in a death penalty cannot be resolved through a court. The Constitution also allows the filing of a review. A death sentence cannot be executed on the basis of the abridged verdict only. The convict has to receive a copy of the verdict. Even the President needs to have the copy of the full verdict to evaluate clemency decision.

Asia Cup set for February 24-March 7 in Bangladesh

The Asia Cup will be held between February 24 and March 7 in Bangladesh, pending confirmation by Nimbus, which owns the broadcasting rights for the event. The dates along with the venues and other details of the tournament were discussed at the Asian Cricket Council meeting in Chennai on Saturday. The meeting was attended by the BCCI president N Srinivasan and representatives from Sri Lanka (Nishanta Ranatuga, Sri Lankan Cricket secretary) and Pakistan (Subhan Ahmed, Pakistan Cricket Board Chief operating officer) as well as ACC chief executive Syed Ashraful Huq.
"India has confirmed those dates. Now we will wait for the broadcaster to confirm they are fine," an official privy to the meeting told ESPNcricinfo. "Those are the only dates when all the four participating countries are available to play."
Earlier the participation of India was in doubt, with the Asia Cup beginning too soon after the finish of the 2014 New Zealand tour as per the original schedules. But the BCCI worked out a revised itinerary for the New Zealand tour-five ODIs and two Tests will be played between January 19 and February 18. "The BCCI said because we want to play the Asia Cup, we have cut down on the New Zealand tour," the official said.
The perceived doubts about Nimbus emerge from the company having filed for bankruptcy. However, the official was confident Nimbus would not pull out because the broadcaster had already paid the first installment of the bank guarantee. "There is no reason why they should not broadcaster had already paid the first installment of the bank guarantee. "There is no reason why they should not broadcast. We will provide them with a cut-off date, otherwise we will have to approach another broadcaster," he said.
Dhaka and Fatullah are likely to host the Asia Cup matches but a final decision on whether the tournament will be played at one or both venues will be taken in the near future. The tournament had been moved to Bangladesh after original hosts India said they were unable to organise the tournament due to a packed international calendar, forcing the ACC to shift the venue at a meeting on April 8 in kuala Lumpur .

Grameenphone wins 3G bid

Bangladesh sold third generation mobile phone spectrum to four of the country's mobile telephone operators for a total of $525 million at a much-awaited auction on Sunday, the telecoms regulator said.
Top operator Grameenphone, which is majority owned by Norway's Telenor, won the auction to acquire 10 megahertz of spectrum for $210 million.

Egyptian Orascom Telecom's Banglalink, Robi, a joint venture between Malaysia's Axiata Group and Japan's NTT DoCoMo, and Airtel, majority owned by India's Bharti Airtel, won bids for 5 megahertz of spectrum each at a price of $105 million.
The upgrade to a faster 3G network was needed to widen Internet access in Bangladesh, one of the world's fastest-growing telecoms markets, said Sunil Kanti Bose, chairman of the Bangladesh Telecommunication Regulatory Commission.
The operators have to pay 60 percent of the total price of spectrum as the first installment within a month and the balance within the next three months, he said.
State-owned mobile operator Teletalk last year launched the 3G service on a test basis. Teletalk will have to pay $210 million for 10 megahertz.
Another operator, Citycell, a joint venture between Pacific Bangladesh Telecom Limited and Singapore Telecommunications, was out of the auction after failing to deposit the $20 million earnest money.
The country's six mobile phone operators have a total of 107 million users while internet services lag far behind with nearly 40,000 subscribers.
